• $120-130 per F2F Visit(s), plus hourly rate for admin. Must have hospice experience

Responsibilities

• Communicate significant findings, problems, changes in condition or environment to the clinical manager/attending physician and other members of the interdisciplinary team.
• Complete face-to-face encounters assessing disease-related changes, decline in function, adherence and effectiveness of pain and symptom management, and the patient and family’s coping mechanisms and quality of life.
• Order and review appropriate lab tests, x-rays, EKGs, and other diagnostics for palliation and treatment of the terminal disease, document the treatment plan, and review results with the collaborating physician.
• Make referrals to other disciplines as indicated by the patient's needs.
• Actively participate in interdisciplinary team conferences.

Qualifications

• Current and unencumbered Nurse Practitioner license in the state of employment.
• Minimum of one (1) year experience as a Nurse Practitioner or as a hospice Registered Nurse within the last three (3) years — REQUIRED.
• Authority to practice and prescribe as a nurse practitioner in the state of service.
• Obtained Medicare billing privileges as a nurse practitioner.
• Meet state-specific physician-nurse practitioner collaborative requirements and provide active agreements.
• Insurance coverage as required by state law for nurse practitioners, or group coverage through employer.
• Knowledge of Medicare, Medicaid, and other regulatory reimbursement and coding requirements.
• Current CPR certification.

Our compensation varies depending on location, knowledge, skills, and experience, reflecting labor costs across U.S. markets.
