Some of your Licensed Practical Nurse (LPN) Job

Responsibilities include:

• Admissions,Discharge and Transfers of residents
• Medicationrequisitions with pharmacy and physicians
• Medication administration
• Followinginfection control, emergencies, and other center policies
• Collaborating with resident’s interdisciplinary team to provide the best outcomes
• Identifying and reporting all abnormal changes in residents’ condition to properpersonnel per center policy
• Worksunder direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.
• Deliversnursing care to patients/residents requiring long-term or rehabilitative care.
• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.
• According to state-specific regulations, implements the patient/resident plan ofcare and evaluates the patient/resident response.
• In accordance with state-specific regulations, directs and supervises caregiven by other nursing personnel in selected situations.
• Maintainsknowledge of necessary documentation requirements.
• Maintainsknowledge of equipment set-up, maintenance and use, i.e., monitors,infusion devices, drain devices, etc.
• Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.

• LPN (Licensed Practical Nurse) licensed by the State
• CPR Certification required
• Two Years prior nursing experience preferred
