Nurse Practitioner​/Physician Assistant - Primary Care

Position Overview

• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities.
• Full-time providers care for patients Monday‑Thursday; optional clinical or administrative day on Fridays.
• Manage a panel of 200 patients (approximately 12‑16 patients per day with 14 being the average).
• Ability to design and manage your schedule.
• Develop and implement care plans appropriate to patients and their desires.
• Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers.
• Access to diagnostics such as labs and x-rays as needed.
• Collaborate with community partners within home care, hospice, and other resources as appropriate.

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program.
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA.
• Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice.
• Ability to obtain and/or maintain DEA certification for the State of required of work.
• Proven ability to communicate effectively with strong verbal skills.
• Excellent interpersonal and customer service skills.
• Must have a valid and unrestricted driver’s license and minimum state‑required liability auto insurance; local travel is required.

• 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant.

• Compensation Base + Quality/Productivity: $125k - 170k

$125,000 - $170,000 USD
