The position requires working Saturdays and Sundays, 6:00AM – 6:00PM, and every other Wednesday. You will deliver one‑on‑one pediatric care in patients’ homes, including responsibilities such as G‑tube care, BiPAP ventilator support, seizure and catheter management, and care of teen patients.
Incentive bonus:
Earn up to $300 – $150 after the first 120 hours worked and an additional $150 after the next 120 hours. The bonus applies to nurses who accept and consistently work scheduled hours on this case.
- Provide comprehensive nursing services in the home setting.
- Maintain accurate, timely clinical records for each patient.
- Evaluate and improve nursing service effectiveness and consult with physicians on care plan changes.
- Report all changes in care to the nurse supervisor.
- Document changes in patient care and support the supervisory nurse.
- Mentor or coach new employees during patient‑specific orientations.
- Educate patients and caregivers on care needs and related topics.
- Participate in FFHC’s quality assurance and performance improvement program (QAPI).
- Attend and complete all mandatory training and in‑services.
- Submit required personnel file documentation promptly.
- Adhere to compliance, ethics, and confidentiality requirements.
- Maintain professional, supportive, and responsive communication.
- Perform additional duties as assigned.
- LPN/LVN nursing degree and unencumbered LPN/LVN license (or equivalent).
- Minimum one year of demonstrated experience in a home‑care or pediatric setting.
- Valid driver’s license with proof of insurance (where required).
- Clear background screening.
- Working knowledge of state Nurse Practice Acts.
- Valid CPR card with in‑person instructor‑led skills check.
- Clear HepB and TB screening (where required).
- Ability to read, speak, and write English.
- Capacity to lift up to 35pounds without assistance.
- Prepared to travel to patients’ homes.
- State‑specific license requirements met as per Florida, Illinois, PA, TX.
- Additional background checks (e.g., child abuse, FBI) where applicable.
- Experience with pediatric or G‑tube care.
