Position: EXPERIENCED RN/LPN Pediatric Home Care – Urgent Cases | Davenport, FL

Urgent Cases (High Acuity)
  • Davenport Urgent:
    Sat & Sun Days (6A–6P) – Grade schooler – G‑tube + Trach – Non‑ambulatory + non‑verbal
  • Davenport Urgent:
    Mon & Fri Nights (7P–7A) – Infant – G‑tube + Partial Trisomy
  • Haines City Weekend Days/Nights Available – Adolescent – Trach + Vent
  • Mon–Fri School Nursing (8:30A–2:30P) – Preschooler – Cardiac issues + ADHD + hyperactivity
  • Lake Wales Day & Night Shifts Available – Infant – BiPAP + G‑tube
Moderate Acuity
  • Plant City Urgent:
    Weekend Days & Nights – Infant – G‑tube + aspiration precautions – Spanish‑speaking preferred – Hospital hold currently
Requirements
  • Active Florida RN or LPN license
  • Pediatric or home care experience preferred
  • Trach/Vent experience required for high‑acuity cases
  • Reliable, compassionate, and patient‑focused
Key Responsibilities
  • Accountable for providing care that is based on licensure/certification within scope of practice
  • Maintains an updated clinical record on each patient at all times, meeting required deadlines for documentation
  • Evaluates the effectiveness of nursing service to the patient and family on an ongoing basis
  • Consults with physicians regarding alterations of patient care plans, documents changes in care, and reports all changes to the nurse supervisor
  • Supports FFHC’s mission by mentoring and/or coaching new employees during patient‑specific orientations
  • Educates patients and their caregivers on topics related to patients’ care needs
  • Participates in FFHC Quality Assurance, Performance Improvement program (QAPI) as requested
  • Continually strives to improve nursing care skills
  • Attends and/or completes all mandatory training/in‑services as assigned
  • Submits required personnel file documentation in a timely manner
  • Adheres to FFHC compliance and ethics expectations
  • Maintains patient and family confidentiality
  • Maintains professional, supportive, and responsive interpersonal communication skills
  • Performs other duties as assigned
Role Qualifications
  • LPN/LVN Nursing Degree, unencumbered LPN/LVN license in state of employment or multi‑state license through Enhanced Nurse Licensure Compact
  • At least 1 year of demonstrated experience
  • Valid driver license with proof of insurance in applicable states
  • Clear background screening
  • Working knowledge of Nurse Practice Acts
  • Valid and current CPR card/certificate with in‑person instructor‑led hands‑on skills check
  • Clear Hep B Screening in applicable states
  • Clear TB Screening in applicable states

Working Conditions and Equipment Use

Work is performed in the patient’s home; not substantially exposed to adverse environmental conditions. Training may be performed in an office setting. Must be able to lift up to thirty‑five pounds (35 lbs) without an assisted lifting device. Frequent use of office machines to include telephone, computer, and printer.
