Home visits in assigned service area:
Polk County surrounding areas

• Oversee the activities of the multi‑disciplinary team that provides care to the patient
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines
• Ensure that all visits scheduled and provided are authorized and covered by physician orders
• Review patient visits weekly and provide plan to Patient Care Supervisor and team assistant, including anticipated weekend visits
• Inform scheduler of any schedule changes and communicate caseload and patient care issues to Home Care Supervisor

• BLS (Basic Life Support)
• RN (Registered Nurse) License
• Driver’s License – State of Florida (Florida driver’s license required within 30 days of hire)
• Required Associate’s or Diploma Nursing
• Required 1 year Registered Nurse experience
• Preferred Bachelor’s Nursing
• Preferred 1 year Home Care experience

This position requires Level II screening through the Florida Care Provider Background Screening Clearinghouse, managed by the Agency for Health Care Administration (AHCA). More information can be found at
