We’re redefining senior primary care in Lakeland, Florida. As a nurse practitioner or physician assistant at Conviva, you will work within a collaborative, multidisciplinary team focused on seniors.
Role & ResponsibilitiesYou will provide comprehensive care to seniors following clinical standards, collaborate daily with your care team to ensure coordinated, high‑quality care, manage referrals and care coordination (hospital/SNF, DME, home health), participate in daily huddles, document care with support from quality‑based coders, support on‑call rotation, and ensure compliance with licensing and accreditation requirements.
Required Qualifications- Master’s Degree as a nurse practitioner or completion of a physician assistant program with board certification.
- Active, unrestricted NP or PA license in the state of practice.
- Two years of experience in value‑based care, geriatric or primary care, managing adults aged 65+ with chronic conditions.
- Computer proficiency (EMR, email, etc.).
- Compliance with Humana/Senior Bridge's Tuberculosis (TB) screening program – you must be screened for TB if selected.
- Experience in Medicare Advantage settings.
- Active DEA license and Medicare/Medicaid provider numbers, or ability to obtain them.
- Familiarity with HEDIS quality indicators and Medicare guidelines.

Hours:
40 - Pay Range: $123,800 - $170,300 per year
- Job is eligible for a bonus incentive plan based on company and/or individual performance.
