RN Hospice Nurse
- Home Visits

Position is based in Lakeland, FL and requires daily travel to assigned areas in Polk County.

Schedule:

• Perform initial nursing assessment on new admissions
• Complete, coordinate and revise the total plan of care and maintain continuity of patient care by collaborating with appropriate staff
• Regularly communicate patient progress to the clinical manager, physician and care team
• Develop, prepare and maintain individualized patient care progress records with accuracy in a timely manner and according to care center policies
• Record pain/symptom management changes and outcomes as appropriate
• Educate families on patient condition, needs and care
• Make referrals to other hospice care disciplines as needed or document rationale for not doing so
• Perform on‑call responsibilities and services to patients/families as assigned
• Perform other duties as assigned

• Current RN license in the state(s) where you will work
• Minimum 1 year of clinical RN experience in an acute healthcare setting
• Current CPR certification

• Hospice or home health experience
• Strong communication and interpersonal skills
• Ability to work independently and manage multiple priorities
• Comfortable with daily travel between facilities
• Proficiency in electronic medical record (EMR) systems
