• Communicate significant findings, problems, changes in condition or environment to the clinical manager/attending physician and other members of the interdisciplinary team.
• Complete face‑to‑face encounters and assess disease‑related changes, decline in function, adherence to pain and symptom regimens, family coping mechanisms, and quality of life.
• Order and review appropriate labs, X‑rays, EKGs, and other diagnostics for palliation and treatment of the terminal disease as dictated by the patient’s condition; document the treatment plan and review results with the collaborating physician.
• Make referrals to other disciplines as indicated by the patient’s needs.
• Actively participate in interdisciplinary team conferences.

• Current and unencumbered Nurse Practitioner license in the state where services are provided.
• Minimum of one year experience as a Nurse Practitioner or as a hospice Registered Nurse within the last three years.
• Authority to practice and prescribe as a Nurse Practitioner in the state where services are provided.
• Medicare billing privileges as a Nurse Practitioner.
• Meet physician‑nurse practitioner restrictive collaborative requirements specific to state; supply all active collaborating physician agreements and indicate whether these will be transferable to employment with Amedisys.
• Insurance required by state law for Nurse Practitioners or eligibility for such insurance through group coverage.
• Knowledge of Medicare reimbursement and coding for all levels of service (home, office, nursing home, assisted living, etc.), Medicaid and other regulatory requirements.
• Current CPR certification.
• Strong communication and interpersonal skills.
• Ability to work independently and manage multiple priorities.
• Comfortable with daily travel between facilities.
• Proficient in electronic medical record (EMR) systems.
• Required background screening through the Florida Care Provider Background Screening Clearinghouse.
