Chapters Health seeks an RN Case Manager to assess patient needs, coordinate the Plan of Care with the IDT, and provide clinical and palliative care to keep patients in their homes. You will document in the EMR and communicate with the IDT to ensure timely visits and appropriate services.
The role requires Florida RN licensure, at least 1 year nursing experience (hospice/hospital preferred), BLS, and the ability to visit patients at home with reliable transportation.
