A concierge-style geriatric practice in the Lakeland area is seeking a Nurse Practitioner to provide high-quality, individualized care for a small, dedicated patient panel. This role offers strong clinical autonomy, a balanced mix of home visits and the chance to help shape a growing practice.

• Conduct comprehensive in-home geriatric assessments
• Provide ongoing care and follow-up visits through home appointments and telehealth
• Collaborate with the physician team as needed while maintaining a high level of independence
• Support care planning, medication management, and chronic disease oversight
• Maintain accurate documentation and utilize EMR/telehealth tools

• Concierge model with a limited patient panel
• High-touch, relationship-based care
• Strong autonomy in clinical decision-making
• Competitive compensation with growth potential as the practice expands

• Active Florida Nurse Practitioner license
• Experience or strong interest in geriatrics
• Comfortable conducting in-home visits throughout Lakeland
• Organized, professional, and confident using technology/telehealth tools
