RN Nurse, Clinical Nurse Specialist
Position Summary
The Clinical Supervisor is responsible for overseeing the clinical operations and delivery of high-quality, patient-centered home health services. This position provides clinical leadership and support to nurses, therapists, home health aides, and other field staff while ensuring compliance with federal and state regulations, Medicare requirements, agency policies, and quality standards.
The Clinical Supervisor works closely with physicians, patients, families, caregivers, and interdisciplinary team members to promote positive patient outcomes, continuity of care, and safe, effective service delivery.
Essential ResponsibilitiesProvide clinical oversight and supervision of home health clinical staff.
Review and approve patient admissions, recertifications, care plans, orders, and changes in condition as appropriate.
Ensure patients receive services according to their individualized plans of care and physician/provider orders.
Monitor clinical documentation for accuracy, completeness, timeliness, and regulatory compliance.
Conduct clinical record reviews and identify opportunities for improvement.
Provide guidance to field clinicians regarding patient assessments, care planning, interventions, and changes in condition.
Monitor patient outcomes, hospitalizations, emergency room visits, rehospitalizations, and other quality indicators.
Participate in case conferences and interdisciplinary team meetings.
Coordinate communication among clinicians, physicians, patients, families, and other healthcare providers.
Assist with scheduling and staffing to ensure appropriate clinical coverage.
Participate in orientation, competency assessment, education, and ongoing training of clinical staff.
Address performance concerns and provide coaching and corrective guidance when necessary.
Assist with incident/complaint investigations and implementation of corrective actions.
Support infection prevention and control activities and agency quality improvement initiatives.
Ensure compliance with Medicare Conditions of Participation, state regulations, payer requirements, and agency policies.
Participate in survey readiness, audits, quality assurance, and performance improvement activities.
Assist with development, implementation, and monitoring of clinical policies and procedures.
Maintain confidentiality and protect patient health information in accordance with HIPAA and applicable regulations.
Perform field visits or patient assessments when clinically indicated or required by agency policy.
Serve as a clinical resource and leader for the home health team.
Current, unrestricted Registered Nurse (RN) license in the state of practice.
Associate or Bachelor’s degree in Nursing; BSN preferred.
Minimum of 2–3 years of clinical nursing experience.
Home health experience preferred.
Strong knowledge of Medicare home health requirements and applicable state regulations.
Demonstrated ability to supervise, coach, and support clinical staff.
Excellent clinical assessment, critical-thinking, communication, and organizational skills.
Ability to work independently and manage multiple priorities.
Proficiency with electronic medical records and clinical documentation systems.
Valid driver’s license and reliable transportation, when required for field responsibilities.
