This position will require the employee to work with Program Participants and is responsible for assessing, linking, coordinating, following up, and advocating for recipients who have experienced domestic violence or sexual violence. The Care Manager provides ongoing counseling and advocacy services to empower and support program participants to start a new and continued life free of abuse. The Care Manager works with individual participants to assess the lethality of the situation and develop a safety plans, assess the program participant's individual strengths, and needs, identifies barriers, and challenges present, offers options, and assists the participants in selecting from the options available.

The Care Manager develops and sustains ongoing relationships with clients served while creating safe and healthy boundaries to maintain ongoing support for the clients and the program. Provide presentations within the community in addition to the DV/VS team.

  • Coordination of services and payments to assist program participants to relocate to a safe place away from the dangers of abusive factors in their lives and accessing community resources.
  • Establish and maintain effective working relationships among a widely diverse group of program participants, staff, agencies, and the community.
  • Works with individual participants to assess the lethality of the situation and develop a safety plan. Assess the program participant's individual strengths and needs, identifies strengths, and challenges present, offers options, and assists the participants in selecting from the options available. Provides on-going safety planning with participants.
  • Has a comprehensive understanding of the dynamics of domestic violence, service programs, confidentiality, and privilege.
  • Documents all key correspondence and communication with the program participants and all partners.
  • Provide monthly updates to supervisor.
  • Attend agency meetings and all other meetings as assigned.

    Provides program participants with status updates, keeping them abreast of issues and solutions that arise.
  • Provides crisis intervention counseling and advocacy support services to all program participants.
  • Maintains data, participant files, records, receipts, invoices, leases, and prepares weekly and monthly reports, weekly time sheet submission and other necessary data related to the relocation program.
  • Supports an environment based in respect and the utilization of ethical communication.
  • Adhere to company finance and accounting procedures related to providing relocation support.
  • Arrive/report to work on time and ready to work.

  • Provide for the special needs of participants at the Shelter.
  • Develop and maintain partnerships in the community relative to employment, housing and financial organizations.
  • Comply with reporting requirements designated by Peace River Center's contractual obligations.
  • Develop & deliver curriculum for programs aimed toward financial stability.
  • Provide housing and job information to participants.
  • Maintain current & updated resource manual for housing, etc. in the tri-county area for use by staff & participants.
  • Coordinate with supervisor new procedures and paperwork to ensure the smooth and efficient operation of programs.
  • Follow up with participants at 30, 60 & 90 days after leaving shelter to assess safety and ongoing needs, if any.
  • Provide information, referrals, and counseling to survivors of domestic violence and the public.
  • Provide transportation for shelter participants, as needed.
  • Provide shift coverage at the Lakeland/Sebring Domestic Violence Shelter, as needed.
  • Shift assignments will be flexible with occasional evening and weekend work required and occasional need to fill in on other shifts due to vacancies, illnesses and vacations.
