The Care Manager provides care management assistance and advocacy services to empower and support program participants to start a new life free of abuse. Works with individual participants to assess the lethality of the situation and develop a safety plan. Assess the program participant's individual strengths and needs, identifies strengths and challenges present, offers options, and assists the participants in selecting from the options available.
This is a non-degreed position that will require the employee to provide support to participants at shelter, outreach office and/or community. Assist with facilitating support groups within the shelter and community, complete intake paperwork and service documentation for survivors of domestic violence who receive victim services with Peace River Center. Provide transportation for participants as needed.

Works with individual participants to assess the lethality of the situation and develop a safety plan. Assess the program participant's individual strengths and needs, identifies strengths, and challenges present, offers options, and assists the participants in selecting from the options available. Provides on-going safety planning with participants.
Meets with participants on a weekly basis. (Shelter-2-3x minimum)
Outreach Specialist-weekly phone contact with participants; 2-3x monthly community visits.
Transition planning with participants for continuity of care when exiting shelter. Follow up contact at 30, 60 & 90 days after exit to assess safety and ongoing needs, if any. Coordinate ongoing support with Outreach Care Specialist.
Has a comprehensive understanding of the dynamics of domestic violence, service programs, confidentiality, and privilege.
Documents all key correspondence and communication with the program participants and all partners in accordance with Community Wellness Grant guidelines.
Provide monthly updates to supervisor.
Attends agency meetings and all other meetings as assigned.
Provides program participants with status updates, keeping them abreast of issues and solutions that arise.
Provides crisis intervention counseling and advocacy support services to all program participants.
Maintains data, participant files, records, receipts, etc. and prepares weekly and monthly reports, weekly time sheet submission and other necessary data related to the Community Wellness Grant.
Adheres to Peace River Center policies and procedures; supports an environment based in respect and the utilization of ethical communication.
Ability to adhere to company finance and accounting procedures related to providing relocation support.
Ability to arrive/report to work on time and ready to work.

Know and understand relevant Domestic and Sexual Violence Laws and Statutes, participants' rights, and DCF/FCASV Standards as relevant to job position.
Ability to prioritize and successfully complete daily activities.
Attend all scheduled work hours, meetings, training, and other center functions, including on-call as required.
