Hospice RN Case Manager

Join our dedicated team at Hospice Services of Massachusetts, where we strive to make a lasting difference in the lives of patients diagnosed with a life-limiting illness. We are seeking a compassionate and experienced Registered Nurse (RN) to serve as a Hospice Case Manager in our Lakeville, Massachusetts location.

Job Summary

As a Hospice RN Case Manager, you will plan, organize, and direct hospice care to meet the needs of individuals and families within their homes and communities. Weekly pay range of $42 to $52 an hour, depending on experience.

Responsibilities
  • Supervise hospice aides
  • Complete comprehensive assessments of patients and families to determine hospice needs
  • Provide professional nursing care by utilizing all elements of the nursing process
  • Assess and evaluate patient status, writing and initiating plans of care, and regularly re-evaluating patient and family needs
  • Develop care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actions
  • Counsel patients and families in meeting nursing and related needs
  • Provide health care instructions and assist patients with activities of daily living
  • Act as Case Manager when assigned by Clinical Supervisor, coordinating patient care for assigned caseload
Requirements
  • Graduate of an accredited school of nursing
  • One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care
  • Current licensure in the State and CPR certification
  • Bachelor's degree, with one (1) year of hospice care experience preferred
