Job Title: RN Case Manager – Floating Territories | Be the Go-To Nurse + Incentive Pay

Compensation: $95,000 – $130,000 per year plus incentive visits
Location: Greater Twin Cities Metro Area (21-county service area)
Schedule: Full-Time with On-Call Rotation

What You'll Do- Step into territories needing support due to census changes or coverage gaps.
- Provide skilled nursing care, assessments, and care planning.
- Complete OASIS visits and coordinate interdisciplinary care.
- Help maintain continuity and quality across teams.
- Educate patients and families while building trust quickly.
- Travel throughout the metro with flexibility and independence.
- Active Minnesota Registered Nurse (RN) license.
- Strong clinical judgment and ability to adapt quickly in new environments.
- Experience in home health preferred, but not required.
- Confident communicator and problem-solver.
- Reliable transportation and willingness to travel.
