In-Home Health - Nurse Practitioner or Physician Assistant Mankato, MN

Overview

Signify Health, part of CVS Health, is seeking a Full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The role involves visiting patients in their homes to conduct medication and medical history reviews, physical evaluations, and, if ordered by a health plan, additional diagnostic tests. The clinician will use an iPad and an intuitive clinical workflow, and is not required to prescribe medicine, order lab tests, or alter treatment regimens.

• Provide in‑home health evaluations lasting approximately 45 minutes.
• Conduct medication and medical history reviews, physical examinations, and diagnostic tests such as diabetic eye exams and spirometry when ordered.
• Use an iPad and the provided clinical workflow to document visits.
• Travel locally and within licensed states as required.
• Conduct virtual visits as needed.
• Deliver Signify Health diagnostic and preventive services products, including spirometry and digital eye exams.
• Build and maintain trusted relationships with patients to improve health outcomes.

• Active, unrestricted license(s) in the coverage area(s); for NP and PA candidates, completion of 2080 post‑graduate collaborative practice hours in an integrated clinical care setting (plus appropriate state attestation) and board certification.
• Preferred: multi‑state licenses.
• Valid license in the state of Minnesota for NP practitioners, if applicable.

• Salary range: $87,035.00 – $ (based on experience, education, geography, and other factors).
