Make a Meaningful Difference! Grace Hospice is seeking a compassionate and experienced Palliative Care Nurse Practitioner to provide high-quality care and support to patients living with serious illness and their families. This role focuses on symptom management, goals-of-care conversations, patient and family education, and coordination with physicians and the interdisciplinary care team.

The Nurse Practitioner will also help support the continued growth of Grace’s palliative care program through community education, partnerships, quality improvement, and increased access to services.

Salary: $115,000 - $128,000 annually

Key Responsibilities
  • Assess and manage acute and chronic health conditions
  • Provide pain and symptom management
  • Help patients and families identify their goals and care preferences
  • Develop care plans in collaboration with physicians and the interdisciplinary team
  • Prescribe medications and order diagnostic testing within scope of practice
  • Maintain timely and accurate clinical documentation
  • Educate patients, families, healthcare partners, and the community about palliative care
  • Support program growth, quality outcomes, and referral relationships
  • Participate in team meetings and an on‑call rotation, when required
Qualifications
  • Master of Science in Nursing from an accredited institution
  • Current Nurse Practitioner license and national certification through ANCC or AANP
  • Minimum of five years of professional nursing experience
  • Experience with pain management, symptom management, patient education, and care planning
  • Palliative care, hospice, geriatrics, oncology, or intensive care experience preferred
  • ACHPN certification or willingness to obtain preferred
  • Current NPI, DEA registration, and Medicare credentialing
  • Valid driver’s license, automobile insurance, and reliable transportation
  • Strong communication, organization, and technology skills
