What You'll Do:As the LPN Care Coordinator, you'll play an important part in ensuring residents receive exceptional care each day while working within a supportive interdisciplinary team. Your responsibilities include:
- Assist each client to reach and maintain his or her highest level of performance and independence by performing assistance with activities of daily living and implementing scheduled services as needed
- Participates with registered nurses in the development and review of care plans; participates in care conferences
- Assist with the admission process as directed
- Perform safe administration of medications and implementation of treatments and other therapies as directed
- Completing documentation with accuracy and attention to detail
- Supporting Resident Assistants through mentorship, coaching, and positive leadership Managing medication orders, refills, and overall medication processes
- Performing supervisory visits, completing chart reviews, and ensuring compliance with company policies and our Medicare Comprehensive License
- Demonstrate effective written and verbal communication with residents, family members, staff, and other health care providers. Maintain service excellence and a high level of confidentiality.
- Actively participate in agency-wide activities and its continuous quality improvement
- Understand the importance of seeking assistance as needed and demonstrate dependability and capability of following instructions. Adhere to policies, and procedures and participate in the organization's quality improvement program.
- Current Minnesota LPN license
- Strong communication skills with the ability to work professionally and respectfully with staff, residents, and families
- Experience in Assisted Living highly preferred
- Familiarity with Alzheimer's/dementia care is a plus
- Confidence using electronic documentation systems
