Job Title: Director of Health and Wellness
Reports to: Executive Director
FLSA Status: Exempt
Position Scope: The Director of Health and Wellness is responsible for directing the Health and Wellness program of the community. Commits to the standards of Evolve Senior Living and state requirements to ensure optimal resident care. Directs staff, complies with regulatory requirements, and is responsible for the management of the clinical budget.

RESPONSIBILITIES- Supports the mission, core values and goals of the organization, upholding and promoting company culture and vision.
- Creates an environment of accountability through leadership and management practices utilizing the Entrepreneurial Operating System (EOS).
- Provide direct oversight of the delivery of care to the residents of the community to ensure emotional, physical, psychological, and safety needs are met through all program services and activities.
- Recruits, hires, trains, monitors performance, schedules, and supervises staff in accordance with company policy.
- Coordinates with the Executive Director to conduct family/resident care conferences and serves as a direct liaison between residents, families, and staff. Communicates with residents, families, and staff to ensure residents are at appropriate levels of care.
- Monitors documentations systems to ensure all regulatory time frames for review of assessments and service plans are met and that resident needs are met. Writes and reviews residents’ assessments and care plans with appropriate staff.
- Assesses and evaluates potential residents for admission into the community and to determine appropriate levels of care. Assessments are done timely and professionally and may require working with the Sales Team to coordinate. Assessments may be done onsite at the community or in the home of the potential resident or in another healthcare setting. Must be able to drive to the location to perform the assessment.
- Plans, schedules, and monitors staff training programs and in-services including statutory or regulator required training. Attends all required training, in-service, and staff meetings.
- Manages and coordinates the work of all nursing and clinical staff, communicates pertinent resident health and safety information. This may include overseeing or completing staff schedules and assignments.
- Manages labor costs, supplies, and other line items in the clinical budget.
- Promotes and protects residents’ rights and treats residents with dignity and respect.
- Demonstrates the ability to remain calm under stressful conditions.
- Represents the property in dealing with outside agencies, including governmental agencies. Participates in property survey inspections made by authorized government agencies.
- Assists and performs in mock surveys.
- Develops a plan of correction of deficiencies in the Health and Wellness Program noted during survey inspection.
- Maintains a safe work environment through the prevention of accidents, preservation of equipment, and achievement of safe working practices.
- Ensures compliance with all state regulatory requirements including mandatory reporting to the state. Responsible for ensuring all incident reports are completed, filed, and followed-up in a timely manner.
