We are expanding our clinical team and are seeking for a Psychiatric Mental Health Nurse Practitioner (PMHNP) who is compassionate, motivated, and committed to supporting the behavioral health needs of residents in nursing home settings across New York and New Jersey . This role is ideal for a practitioner who is passionate about geriatric mental health and eager to make a meaningful impact on the quality of life of older adults.
RequirementsKey Responsibilities
- Conduct comprehensive psychiatric evaluations and ongoing mental health assessments for geriatric patients in skilled nursing facilities.
- Develop individualized and evidence-based treatment plans tailored to each patient’s diagnosis and care needs.
- Oversee medication management, including prescribing, monitoring, and adjusting psychotropic medications.
- Work collaboratively with an interdisciplinary team—physicians, therapists, nurses, social workers—to ensure coordinated and holistic patient care.
- Provide therapeutic support and counseling to patients and, when appropriate, their families.
- Educate patients and family members on mental health conditions, treatment pathways, and healthy coping strategies.
- Document clinical encounters accurately and maintain compliance with regulatory and organizational charting standards.
- Participate in team meetings, case reviews, and initiatives aimed at enhancing care delivery and clinical quality.
- Active New Jersey Nurse Practitioner license with certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) .
- Ability to work autonomously while effectively managing a diverse patient caseload.
- Strong clinical assessment skills and solid knowledge of psychiatric medications and treatment options.
- Excellent communication and interpersonal abilities to build trust with patients, families, and colleagues.
- Competency with electronic health record (EHR) platforms and basic computer programs.
