About the Role
Tru Care Management is seeking motivated and detail-oriented Certified Nursing Assistants (CNAs) to join our growing Chronic Care Management (CCM) team. This is a non-clinical, in-office position focused on coordinating care for patients with chronic conditions.
You will play a key role in improving patient outcomes by maintaining consistent communication, tracking health data, and supporting providers with documentation and care planning.
Key Responsibilities- Conduct monthly patient outreach via phone
- Document all patient interactions in the EMR system
- Track and manage patient care plans
- Coordinate with providers, nurses, and facility staff
- Identify changes in patient condition and elevate when appropriate
- Ensure compliance with CMS guidelines for CCM services
- Assist with care gap identification and follow-ups
- Active CNA certification (required)
- Strong communication and interpersonal skills
- Comfortable working on phones for extended periods
- Basic computer skills (EMR experience preferred)
- Highly organized and detail-oriented
- Ability to work in a fast‑paced, team environment
- In-office, structured schedule
- No physical bedside care required
- Focus on communication, coordination, and documentation
- No lifting, bathing, or direct patient care tasks
