RN Care Coordinator

Job Summary and Responsibilities

The RN Care Coordinator is responsible for overseeing the progression of care and discharge planning for patients requiring these services. The role focuses on care coordination, communication and collaboration with utilization management, nursing, physicians, ancillary departments, insurers and post‑acute service providers to progress the care toward optimal outcomes at the appropriate level of care. The RN Care Coordinator advocates for the patient and family by identifying and addressing patient choice, spiritual needs, cultural, language and socioeconomic barriers to care transitions, and strives to enhance the patient experience.

RN Case Managers may be eligible for a relocation bonus up to $8,000.

• Graduate of an Accredited School of Nursing
• Nursing Diploma or Associates Degree in Nursing required; BSN preferred
• RN License in applicable state of employment required
• Minimum two (2) years of acute hospital clinical experience or a Master’s degree in Case Management or Nursing field in lieu of acute experience
• At least five years of total nursing experience required
• Certified Case Manager (CCM), Accredited Case Manager (ACM‑RN), or UM Certification preferred
• Experience working with EMR preferred
• Working knowledge of regulatory requirements and accreditation standards strongly preferred
• BLS from the American Heart Association required within three months of hire if located within hospital

Medium work: exert up to 50 lbs force occasionally, up to 20 lbs frequently, and up to 10 lbs constantly.

Located in Lakewood, Colorado, St. Anthony Hospital is a Level I Trauma Center providing a full range of medical specialties and health‑care services.

$34.14 – $61.23 per hour
