About the Position

The Pueblo Regional Center (PRC) provides a safe, secure, therapeutic setting for comprehensive care of individuals with developmental disabilities and medical issues. Staff serve 24‑hour nursing care, support therapy, medication management, discharge planning, and community integration. The role emphasizes stabilization, knowledge of treatment options, and person‑centered services.

Lead Work Responsibilities
  • Provides leadership of direct care staff and ensures duties are performed within guidelines.
  • Hands‑on care or instructions in daily living activities for individuals receiving services.
  • Monitors the milieu for interventions, plans community outings, and integration opportunities.
  • Delivers services as outlined in individual plans, directs staff, and ensures compliance with schedules.
  • Provides onsite training, counseling, and monitoring of support staff.
  • Reports problems such as excessive call‑offs, missed swipes, or tardiness to supervisor.
  • Identifies training needs and requests professional staff training.
  • Collects, completes, monitors, evaluates, and delegates administrative forms and client records.
  • Serves on rotation for on‑call services.
  • Follows up on medication variances when necessary.
  • Completes inventory of the assigned home and residents per policy.
  • Maintains communication log, daily living reports, policies, procedures, and orientation book.
  • Acts as supervisor if the Residential Coordinator is unavailable.
  • Replaces other locations or shifts as needed.
  • Performs other duties as assigned.
Direct Care Services
  • Physically lifts, transports, and/or positions individuals using approved lifting techniques.
  • Assists residents with eating, hygiene, dressing, bathing, toileting, and clothing.
  • Operates a motor vehicle for transportation to campus or external sites.
  • Assists with personal shopping and ordering supplies.
  • Responds to behavioral challenges using specific intervention programs.
  • Participates in interdisciplinary team discussions and formulates accurate assessments.
  • Maintains a well‑maintained living site and assists in decorating personal spaces.
  • Follows policies on restraints, abuse reporting, and confidentiality.
  • Communicates positively with residents, family, visitors, peers, and supervisors.
  • Uses State E‑mail, Sign Me Book, communication logs, and Individual Plans as resources.
Clinical Skills
  • Provides nursing care and supports, advising the RN on behavioral and medical needs.
  • Delivers services per licensure and governing bodies, fostering independence.
  • Familiarizes with allergies, sensitivities, and nutritional needs.
  • Administers ordered medications, reports variances, and consults RN as needed.
  • Documents treatment outcomes and program implementation accurately.
  • Records medication timing, resident responses, intake/output, and other medical data.
  • Supervises staff on administering medications, treatments, and dietary orders.
  • Follows infection control standards and nursing competencies.
  • Maintains required licensure and certifications.
Safety & Security
  • Uses intervention and verbal techniques for behaviorally troubled individuals.
  • Completes incident reports, injury reports, debriefing papers, and progress notes.
  • Obtains emergency assistance and supports oxygen or laboratory services.
  • Participates in fire and code zero drills and documents findings.
Department Standards
  • Follows policies on pharmacy, infection control, physician orders, court orders, and regulations.
  • Adheres to nursing standards and Colorado Practice Acts.
  • Participates in agency committees, meetings, in‑services, and training sessions.
  • Attends mandatory annual training and assigned course requirements.
  • Reports abuse or harassment to the Emergency On‑Call Personnel and QA.
  • Engages in the QAPI process and documents outcomes.
Education, Licensure, and Experience
  • High school diploma or General Education Diploma (GED).
  • Two (2) years of experience in an occupational field related to the position.
  • Certification or licensure appropriate to the work assigned.
