Responsibilities
- Perform care management activities for members with complex and chronic care needs
- Ensure high quality, cost-effective care and appropriateness of treatment
- Partner with physicians and/or pharmacists to interpret appropriateness of care
- Collaborate with providers to assess consumer needs and plan for adverse events
- Conduct clinical assessments to develop individual care plans and goals
- Monitor and evaluate effectiveness of care management plans
- Requires an ASN or ADN and a minimum of 3 years of clinical experience
- Current, valid, active unrestricted RN license in applicable state(s) required
- Multi-state licensure is required for services in multiple states
- Preferred: BSN, Specialty Pharmacy experience, Bilingual, Drug Manufacturer experience, Telephonic Nursing experience
