Hope West PACE is seeking a Primary care or Geriatric experienced Nurse Practitioner to join a robust interdisciplinary team, to collaborate in developing and implementing comprehensive care plans while enhancing the lives of medically complex elderly individuals.
Hope West PACE participants receive wrap around services that include but not limited to in home care services, transportation to a Hope West PACE center for primary medical care, social and recreational activities.
