Physical Therapist (PT) - Skilled Nursing Facility
RCM Health Care Services is seeking dedicated and compassionate Physical Therapists (PT) to join our team to support our Skilled Nursing Facilities (SNF). The ideal candidate will provide high-quality rehabilitative care to residents, helping them regain mobility, strength, and independence. This role involves assessing patient needs, developing individualized treatment plans, and working collaboratively with an interdisciplinary healthcare team.
Full-time 13-Week Positions (chance for extension)!
Physical Therapist

Key Responsibilities:

Conduct comprehensive evaluations to assess patient needs  Develop and implement individualized treatment plans  Provide therapy to enhance mobility, strength, and functional abilities  Collaborate with a multidisciplinary team to ensure holistic care  Maintain accurate and timely documentation of patient progress  Educate patients and families about therapy goals and techniques     Physical Therapist

Qualifications:

Education:

Doctorate (DPT) or Master's Degree in Physical Therapy from an accredited program.
Licensure:  Active  Physical Therapist license  in New York State or eligibility to obtain one.

Experience:

Prior experience in a skilled nursing facility or geriatric rehabilitation preferred but not required.

Skills:

Strong communication, problem-solving, and patient-care skills. Ability to work independently and as part of a team.
