Overview

Hope Human Services is a premier provider of Supported Living services for developmentally & intellectually disabled adults in the State of Washington. The Case Manager participates as a member of the management team in maintaining quality and continuity of care. The person in this position will, along with other members of the management team, provide leadership and guidance; and provide oversight to direct support professionals.

Responsible for staff development; parent and family education; learning individuals care prior to admittance and educating others on care; assisting with housing for individuals and admits. Works collaboratively with all other disciplines to ensure all needs are met for the individuals. Responsible for hiring all direct support professionals, ensuring they are oriented and prepared to fulfill the duties and responsibilities of a direct care position.

Supervises all direct support professionals, including progressive discipline, performance evaluation, competency, and compliance with policies and procedures. Ensures compliance with all state regulations, including but not limited to licensing requirements, for the provision of care. Schedules staff for program and responsible for education and orientation on the individuals plans, and forms for documentation. Participates in rotating, on call responsibilities providing support 24 hours per day, seven days per week.

Responsibilities

  • Hire, orient, supervise, and evaluate direct support professionals; implement progressive discipline and ensure compliance with policies and procedures.
  • Ensure compliance with all state regulations, including licensing requirements, for provision of care.
  • Schedule staff for program, education, and orientation on individual plans and documentation forms.
  • Collaborate with all disciplines to ensure needs of individuals are met; provide leadership and guidance within the management team.
  • Participate in rotating on-call responsibilities to provide 24/7 support.
Qualifications & Essential Job Functions
  • Be at least 21 years of age or older
  • Teaches and mentors staff to provide high level care in a safe environment
  • Demonstrates direct care skills, professionalism, critical thinking, and objectivity
  • Ability to access the residences of the people we support, specifically those in the caseload for the assigned program
  • Minimum high school diploma or GED;
    Associate or Bachelor’s degree in Human Services or related field preferred
  • One or more years of supervisory experience and case management in Supported Living
  • Skills necessary to provide and teach individual care
  • Experience providing support for intellectually and developmentally disabled adults
  • Must have a currently valid driver’s license or be an active duty military personnel or dependent and able to be insured to drive agency vehicles
  • Proof of vehicle insurance
  • Provides driving abstract on annual basis or as requested
  • Provides valid CPR and First Aid or obtain it within 60 days of employment
  • Provides Supported Living letter of exemption or 75-hour training certificate or obtain it within 120 days of employment
  • Provides a peer coaching certificate or obtain it within 180 days of employment
  • Able to pass a criminal background check with the State of Washington and maintain clearance during tenure of employment
  • Ability to use email, word processing and spreadsheet software. Microsoft Office experience preferred.
  • Ability to use Therap or similar program for online documentation of care of individuals supported
