• Establish short- and long-term rehabilitation goals according to an individualized patient treatment plan.
• Measure progress or lack thereof to achieve these goals while working directly with patients.
• Provide therapy treatment to patients in their homes as prescribed by a physician.
• Select appropriate assessment tools and methods to accurately determine the patient’s status for developing an optimal treatment plan.
• Assess the educational needs of patients and families and implement a variety of methods for patient/family education and training during the rehabilitation process.
