LTSS Service Care Manager - J01031
Location:
Lubbock, TX
Manage and coordinate long-term services and supports for individuals. Conduct assessments to determine client needs and develop care plans. Facilitate communication among clients, families, and service providers. Monitor service delivery and ensure compliance with regulations. Provide education and resources to clients and families regarding available services.

- Provide education and resources to clients and families regarding available services.
- Relevant degree in social work, nursing, or related fields. Licensure in the state applicable to the profession required.
- Benefits are available to full-time employees after 90 days of employment.
- A 401(k) with company match is available after 1 year of service.
