Job Objective
Reporting to the Director of Rehabilitation Services, provides for the carrying out of patient referrals under the general direction of the referring physician to support the Direction of Rehabilitation Services, the Physical Therapy Supervisor and the Rehabilitation Services Department.
Duties and ResponsibilitiesA. Provision of Patient Care
- Performs speech and swallow evaluations as ordered by a
- Based on the evaluation, develops treatment programs with appropriate treatment objectives which are mutually agreed upon by the
- Provides for the implementation of proper treatment programs through themselves or through supervision of a physical therapy assistant or aide.
- Periodically evaluates the patient's response to treatment and alters the program as appropriate with the physician's approval.
- Administers standard diagnostic or prognostic tests of speech, swallowing and cognitive functional ability.
- Maintains appropriate patient records including the initial evaluation, daily treatment charting, progress reports, and discharge summary in the patient's medical
- Documentation on the Multidisciplinary Care Plan and Teaching
- Communicates effectively verbally and with written communications to referring physicians.
- Provides timely and accurate billing information specific to patient interventions
- Maintains effective relationships with physicians, physician's offices, other departments within the hospital, coworkers and patients.

- Participation in departmental staff meetings as deemed appropriate by the Director of Rehabilitation Services.
- Demonstration of proper safety practices in the clinic and on the hospital

- Performance of housekeeping tasks in their work

- Participation in continuing education for patients, hospital employees or outside
Skills and Abilities
Knowledge
- Knowledge of medical
- Knowledge of the theory, techniques and practices of accepted speech language, cognitive and swallowing treatments for geriatric, adult, adolescent and child patient
- Knowledge of the English language and possess good composition skills for the purposes of written communication.
- Knowledge of Antelope Valley Medical Center's hospital Information system (HIS).
- Accurate assessment of a patient's progress on the basis of judgmental criteria and measurable or verifiable criteria.
- Training of non-medical persons to assist with a patient's home program/management.
- Good oral and written communication
- Good interpersonal skills and customer
- Communicate effectively while maintaining good working relationships with coworkers, managers, hospital staff, physicians, patients and outside customers.
