Personal Service Coordinator
35 hours per week / $24.00 per hour (compensation based on experience and other factors permitted by law).
SummaryServes as the primary support and case manager for adults with mental illness by assisting them to live as independently as possible in their chosen community.
Essential Duties and Responsibilities- Develops a Personal Service Plan and/or Barrier List with each member that identifies their personal needs and goals.
- Mediates and advocates on behalf of the member to help them obtain and keep housing, health‑care benefits, entitlements, etc.
- Visits members in their place of residence, hospital or jail as needed to help them live successfully and prevent homelessness.
- Transports members to appointments and on errands when appropriate.
- Oversees and monitors members’ individual and group activities both inside the program and out in the community.
- Provides coaching, counseling, mentoring, instruction, training, feedback and positive reinforcement for members in areas such as money management, grooming and hygiene, meal preparation, public transportation and other independent living skills.
- Devises and implements strategies for outreach to members who tend to isolate themselves.
- Knowledgeable in motivational interviewing and harm reduction.
- Supports individuals and team goals in community integration and employment; works with Community Integration to assist members in achieving integration and employment goals.
- Participates in staff and community meetings and in the day‑to‑day operations of the program as needed.
- Required to bill 85 hours per month into the Electronic Health Care System.
- Maintains all records and documentation needed for program compliance and accountability in accordance with MHALA standards and funding source requirements.
Position Requirements
- High School Diploma or GED, plus 2 years’ experience providing psychosocial rehabilitation related services to a disabled population (preferred).
- All billing and supporting documentation shall be entered into MHALA’s EHR within 24 hours from the date of service delivery.
- All required data collection documents (as applicable to this program: MORS, Determinants, KEC, IHOMS, HMIS, etc.) are submitted on or before the due date.
- Demonstrate knowledge of community resources.
- Must function well in a team‑oriented approach to member services.
- Must have a valid CPR certificate.
- Knowledgeable in computer applications such as Microsoft Word, Excel, and Outlook.
- Driving with a current class C driver’s license; a vehicle for transportation, proof of valid automobile insurance, and the ability to be insured by MHALA liability insurance are conditions of employment.
- Flexible work schedule to include some evenings or weekends, and on‑call cell phone services when necessary.
- TB clearance and re‑testing every year are conditions of employment.
