Roles & Responsibilities   Evaluate and assess patients' physical conditions to create clear baselines and measurable goals
Develop and implement individualized treatment plans that help patients regain mobility and independence
Provide therapeutic exercises and mobility training with a hands-on, patient-centered approach
Monitor and document patient progress clearly and thoroughly to support continuity of care
Collaborate with physicians, nurses, and interdisciplinary teams to deliver coordinated treatment
Ensure compliance with facility policies and regulatory standards to maintain high-quality care
Qualifications   Active California Physical Therapist License (required)
Doctor of Physical Therapy (DPT) or equivalent
Current BLS certification
SNF experience preferred but enthusiastic, patient-focused clinicians are encouraged to apply
Strong clinical reasoning and documentation skills
Education   Doctor of Physical Therapy (DPT) or equivalent
Knowledge & Skills   Skilled in patient assessment and developing individualized treatment plans
Experienced with therapeutic exercises and mobility training
Comfortable documenting progress and communicating with interdisciplinary teams
Knowledgeable about SNF workflows, regulatory standards, and quality documentation
Strong interpersonal skills and patient-focused care delivery
