Roles & Responsibilities Implement occupational therapy treatment plans under OT supervision to help residents regain independence
Support residents in improving functional abilities and overall quality of life through compassionate care
Document patient progress accurately and efficiently to maintain continuity of care
Collaborate closely with the rehab and nursing teams to deliver coordinated, person-centered care
Maintain compliance with state and facility guidelines to ensure safe, high-quality services
Qualifications Valid California COTA license (required)
SNF experience preferred, especially in geriatric rehabilitation
Strong interpersonal and clinical skills to build rapport and deliver effective interventions
Ability to adapt and thrive in a fast-paced skilled nursing environment
Comfort working under OT supervision and as part of an interdisciplinary team
Knowledge & Skills Geriatric rehabilitation knowledge and best practices
Skilled documentation and progress note writing
Strong communication and teamwork abilities
Clinical assessment and treatment implementation for functional goals
Adaptability in a dynamic skilled nursing setting
