Candidate Pay Rate:  $90 - $63 per hour

Roles & Responsibilities   Perform comprehensive patient evaluations to inform individualized care
Develop and implement targeted treatment plans for each patient
Provide skilled therapy focused on mobility, balance, gait training, and improving functional independence
Complete timely documentation including evaluations, daily notes, progress reports, and discharge summaries
Collaborate with interdisciplinary team members such as OT, SLP, nursing, and physicians to coordinate care
Ensure compliance with Medicare and facility guidelines
Educate patients and caregivers on home exercise programs and safety strategies to support long-term recovery
Qualifications   Active California Physical Therapist license
Graduate of an accredited Physical Therapy program
Current CPR/BLS certification
SNF or post-acute experience preferred
Strong communication and documentation skills
Ability to manage productivity expectations in a fast-paced setting
Education   Graduate of an accredited Physical Therapy program
Knowledge & Skills   Physical Therapy principles and patient evaluation
Treatment planning and individualized interventions
Mobility training, balance training, and gait training techniques
Documentation best practices and timely clinical notes
Interdisciplinary collaboration and care coordination
Understanding of Medicare and facility compliance expectations
Patient and caregiver education for functional independence
