Position: Anthem Hospice- Full Time RNCM
At Anthem Hospice, we are dedicated to providing compassionate, patient-centered end-of-life care that brings comfort, dignity, and support to patients and their families. We are seeking a caring and experienced Registered Nurse (RN) to join our California hospice team.

As a Hospice RN, you’ll play an essential role in coordinating and delivering individualized care in patients’ homes and other care settings. You’ll work closely with patients, families, physicians, and our interdisciplinary team to manage symptoms, provide education and emotional support, and help ensure each patient receives the highest quality of care throughout their hospice journey.

Key

Duties Include:

Follow Agency Policies and Procedures, Federal and State Regulations and Standards of Practice.

Attend Agency In-services, Case Conference, or other meetings as requested.

Provide skilled nursing services in accordance with the orders of a physician and under a plan of care in a patient’s  responsible for the observation, assessment, nursing diagnosis, counsel, care and health teaching of the ill, injured, or confined, and the maintenance of health and prevention of illness of others. Records the health needs of the patient.

Performs and completes start-of-care assessment, medication profile, initiates Nursing or Home Health Aide plan of care, and/or Homemaker/Companion service plan and associated paperwork in accordance to agency policy.

Evaluates the effects of the care given and regularly reevaluates the patient’s nursing needs.

Evaluates and revises the plan of care, as necessary.

Provides those services requiring substantial specialized nursing skill based on the physician’s orders and the nursing diagnosis. Examples of such services include, but are not limited to; physical assessment, patient and family teaching, urinary catheter insertion.

Foster a positive relationship with patients/caregivers and doctors to improve patient outcomes.

Tactical Deliverables:

Notifies patient/clients prior to visit and communicates further visits scheduled.

Drives responsibly and safely to and from client’s home.

Schedules own time based on priorities and maintains productivity utilizing time management.

Provides appropriate and thorough service to patients/clients, ensuring proper completion of services in an accurate and timely manner and in accordance with agency policy.
