The People Concern is seeking a Case Manager for Scattered Site Housing to join a multidisciplinary team. The role involves travel across the greater Los Angeles area to meet clients and help them attain and sustain permanent housing in partnership with DHS/HSH.
You will assess housing stability, provide stabilization services and coordinate housing, health, mental health, and income resources tailored to each client’s needs. This is a 100% onsite position at our Lancaster/Santa Monica locations.
