• Administer prescribed medications and document administration according to physician orders and facility policy.
• Monitor residents’ conditions, assess for changes, and report deviations to the charge nurse promptly.
• Assist with activities of daily living (ADLs) and provide supportive care to residents to maintain comfort and dignity.
• Obtain and interpret routine vital signs, respond to call lights, and elevate concerns as needed.
• Maintain accurate nursing notes, care plans, and documentation within the electronic or paper charting system.
• Collaborate with the interdisciplinary team to coordinate care plans, safety interventions, and discharge planning when applicable.

• Active Licensed Practical Nurse license in New Hampshire (or eligibility for licensure in NH).
• Prior experience in a long‑term care or skilled nursing setting preferred.
• Ability to work the evening shift and adapt to a rotation as needed.
• High school diploma or equivalent; completion of an approved practical nurse program.
• Ability to meet travel assignment requirements and pass standard pre‑employment screenings.
