• Administer prescribed medications and treatments per physician orders; document accurately
• Monitor residents’ health status, perform routine assessments, and record vital signs
• Provide essential nursing care, wound management, and assistance with activities of daily living
• Maintain clear, timely documentation in the resident care records; ensure regulatory compliance
• Collaborate with the interdisciplinary team to update and implement care plans
• Uphold patient safety, infection control practices, and privacy standards

• Active LPN license to practice in New Hampshire
• Current CPR/BLS certification
• Prior long-term care or skilled nursing experience preferred
• Strong communication and documentation skills; ability to work in a team
• Availability for evening shifts and a 10-week travel assignment
• Ability to work effectively in a long-term care setting and meet assignment requirements
