LVN ​/ LPN – LTC​/SNF

Long-Term Care Nurse

Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting. Ensure compliance with healthcare regulations and deliver high-quality patient care.

Responsibilities:

• Assess patient needs and develop personalized care plans.
• Administer medication and treatments as prescribed by physicians.
• Monitor and document patient health status and progress.
• Collaborate with healthcare team to coordinate effective care.
• Maintain accurate and detailed patient records.
• Adhere to safety protocols and infection control procedures.

Qualifications:

• Valid LVN or LPN license in the state.
• Proven experience in LTC or SNF environments preferred.
• Ability to comply with vaccination and exemption requirements.
• Strong communication and patient care skills.
• Ability to wear a photo badge and bring valid  the facility.
• Commitment to providing compassionate, compliant care.
