Job Summary:

Provides licensed practical nursing care in a long-term care or skilled nursing facility setting. Supports patient needs through direct clinical care, ensuring compliance with healthcare standards and facility policies.

Responsibilities:

• Administer medications and treatments as prescribed, monitoring patient responses.
• Perform clinical assessments and document patient progress accurately.
• Assist with activities of daily living, including hygiene, nutrition, and mobility support.
• Collaborate with healthcare team members to develop and implement care plans.
• Maintain strict adherence to safety protocols and infection control policies.
• Ensure documentation and record-keeping comply with regulatory standards.

Qualifications:

• Hold a valid state license as an LVN or LPN.
• Bring a current COVID-19 vaccination or valid exemption documentation.
• Possess strong clinical assessment and patient care skills.
• Demonstrate excellent communication and teamwork abilities.
• Maintain compliance with facility policies and healthcare regulations.
