The anticipated hourly range for this position is $20.05/hr - $25.07/hr - $27.08/hr final compensation will be determined based on the selected candidate's applicable experience, skills, qualifications, and other job‑related factors.
POSITION SUMMARY:Adhering to S System Values, Service Excellence, and Standards of Behavior, the Inpatient Case Manager leads comprehensive patient care coordination during hospitalization, ensuring safe, timely, and effective discharge while addressing medical, psychosocial, and financial barriers. This role requires clinical judgment, decision‑making, and collaboration with interdisciplinary teams to optimize patient outcomes and continuity of care, especially in resource‑limited rural settings.
ESSENTIAL QUALIFICATIONSEducation:
- An Associate's degree in Nursing, Social Work, Healthcare Management, or a related field is required, or in lieu of formal education, an additional 4 years of experience in the field is acceptable.
- Nurse or Social Worker preferred. Will consider LNA/MA with appropriate experience.
- Education requirements are dependent upon credentials held.
Certification:
- Certification in Case Management (CCM) or equivalent is preferred.
- All employees and contracted staff will have either successful completion of an American Heart Association
* accredited course in Basic Life Support (BLS), or ACLS and PALS combined. (for clinical staff), Heartsaver CPR AED (for nonclinical staff), and renewal on a regular basis.

- Perform in-depth assessments of patients’ medical, psychosocial, and financial needs upon admission.
- Identify potential barriers to discharge (transportation, home support, insurance, medication access).
- Develop individualized care and discharge plans in collaboration with the interdisciplinary team.
- Clinical Decision‑Making & Coordination
- Determine appropriate level of care for patients (inpatient, observation, rehabilitation, home health, skilled nursing).
- Coordinate with physicians, nurses, therapists, and external providers to ensure cohesive care delivery.
- Make recommendations to facilitate safe transitions and prevent readmissions.
