Standard Business Hours - Annual Salary: $80,000 – $110,000 per year
- Sign‑on Bonus: $10,000 (paid in separate installments)

On the road for patient visits or training at the local office or remotely
- Provide physical therapy services according to a written physician’s plan of care and the state Physical Therapy Practice Act.
- Assess and evaluate therapeutic, rehabilitative/functional status.
- Develop, implement, and direct physical therapy programs that focus on rehabilitation.
- Evaluate the patient’s home environment.
- Assess patients for muscle strength, mobility, gait, and ROM potential for rehab.
- Direct patients and families/caregivers in the use and care of therapeutic appliances.
- Determine priority needs for physical therapy.
- Report the patient’s reaction to treatment or changes in their condition to the physician.
- Initiate the physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient.
- Participate in case conferences to discuss the goals of the physical therapy program with other personnel and/or family/caregivers.
- May train the patient in the use of prosthetic devices.
- Identify patient and family/caregiver needs for other home health services and refer as necessary.
- Prepare and submit clinical and progress summaries based on the attainment of goals.
- Participate in discharge planning for the patient.
- Provide physical therapy consultation to home families/caregivers when indicated.
- Provide in‑service education programs for nursing organization personnel as needed.
- Participate in the peer consultation process.
- Supervise PTAs according to organization policy and state regulations.
- When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two weeks.
- Collaborate with other team personnel to achieve well‑rounded care.
- Active physical therapy license in Ohio.
- Bachelor’s, master’s, or doctorate in physical therapy from an accredited program.
- Current CPR certification.
Recent TB test. - Valid driver’s license and reliable transportation with active automobile insurance.
- Previous experience in physical therapy, rehabilitation, or other related areas.
- Two years of appropriate experience as a physical therapist.
- Community/home health experience.
- Good verbal and written communication skills, and strong organizational skills.
- Ability to handle physical workload.
- Ability to build rapport with patients.
- Strong leadership qualities.
