Pay Rate: $23.31-$38.39 hourly

• Collaborates with physicians and other members of the healthcare team to develop patient specific treatment plans.
• Monitors patients clinical progress throughout hospitalization through discharge.
• Provides ongoing assessment and consultation to meet patient and family needs.
• Assesses and provides educational instruction and materials to meet learning needs of patient and family members as related to each case and to healthcare providers relevant to particular cases.
• Completes psychosocial assessments.
• Completes multidisciplinary treatment plan.
• Assists with multidisciplinary assessment summary page for all discharges.
• Provides groups and individualized therapy as directed.
• Facilitates patient transfers to other psychiatric facilities.
• Coordinates with the psychiatric physician facilitation for judicial hearing needs.
• Coordinates post discharge needs.

• Bachelors in Social Work with approval of Ohio Department of Mental Health permitted.
• Masters degree in Social Work preferred.
• Licensed Social Work (LSW) with practice directed and supervised by attending Psychiatrist(s) permitted.
• Licensed Independent Social Worker (LISW) preferred.
• A minimum of three years of experience or combination of experience and education.

Exerting up to 20-50 pounds of force occasionally, and/or up to 10-25 pounds of force frequently, and/or a negligible up to 10 pounds amount of force often.
