Position: Home Health PDN (LPN or RN) - Full-time, Weekdays - Lancaster, OH

We are seeking a highly skilled LPN or RN to join our home health agency as a Private Duty Nurse for an adult client in Lancaster, Ohio on a full-time basis (Weekdays).

This position must be filled immediately:
Schedule Commitment = 40 Hours/week

Required shifts: Monday–Friday, 8 hours per day, with some flexibility on consistent start and end times (approximately 6–8 am start / 6–8 pm end).

Experience:

Must have 1+ years of paid experience in trach, G‑tube care, vent (or Trilogy) care, medication administration and feedings. Male nurses encouraged to apply.

About the Client
  • This adult client has a loving, devoted family with both parents also serving as caregivers, so on‑site support is available.
  • The patient requires total care and support for all daily living needs, including bathing, dressing, feeding, routine maintenance exercises and passive range of motion, transfers, and other incidental services.
  • HCN is also seeking PDNs for two additional shifts to offer total care for this client and his family:
  • PDN Weekend Shift: 24 hrs/week (12 hrs Saturdays + 12 hrs Sundays, with some flexibility on start and end times)
  • PDN Weekend Night Shift: 32 hrs/week (Friday nights: 11 pm–7 am; Saturdays and Sundays: 7 pm–7 am)

Starting Pay Rate
  • LPNs: $28–$30 per hour
  • RNs: $35–$40 per hour
Position Summary

A successful PDN at Home Care Network, Inc. performs skilled nursing functions in accordance with federal, state, and local laws and within the guidelines of their professional organization and agency. All functions are performed in accordance with established policies, the state Nurse Practice Act, and as delegated by the RN Case Manager.

Reports to:

RN Case Manager;
Clinical Administrator/Director of Nursing.

Qualifications
  • Graduate of an accredited school of professional nursing.
  • Current license to practice as an LPN or RN in Ohio.
  • Minimum of one (1) year of experience in a home healthcare setting, acute care setting, or equivalent nursing experience. A passion for providing excellent in‑home care is a must.
  • Experience in trach care, G‑tube care, medication administration, and tube feedings required.
  • Active CPR and First Aid certificates highly desired; agency will provide recertification if required.
  • Good interpersonal communication skills – written and verbal.
  • Current driver's license, good driving record, and reliable transportation.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail‑oriented (in care + documentation), accountable, friendly and accommodating, respectful.
  • Excellent attendance record with past employers.
Essential Functions/Areas of Accountability
  • Provides professional nursing care under the direction of an RN. Communicates appropriate status/condition changes to the RN per agency guidelines.
  • Provides skilled nursing visits for clients as directed by the RN Case Manager. Follows the care plan established by the RN.
  • Recognizes and reports changes in client condition to the Case Manager and physician, as directed.
  • Communicates information in a timely manner, consistent with agency policy and the urgency of the situation. Communication is verbal, as needed, and through complete and accurate documentation in the clinical record.
  • Scope of clinical practice includes personal care services, assessment of findings, medication monitoring and administration, client teaching, emergency intervention, and nursing services within professional scope of practice and as delegated by the Case Manager.
  • Individuals with the license designation of “LPN M‑IV” may perform venipuncture for laboratory analysis.
  • Promotes personal safety and a safe environment for clients and coworkers.
  • Demonstrates knowledge of safety/infection control practices by complying with policies and procedures.
