Flint Ridge Nursing and Rehabilitation, a long-term care facility that specializes in individualized care and rehab for our residents, is seeking a dedicated MDS Nurse. This full-time position will be split between two of our buildings, giving you a unique opportunity to work with diverse teams and residents. • Monitor and guide the completion of assessments in a timely manner.
• Involve and supervise the inputting of MDS assessments, transmission to the state, and receiving receipts of validation.
• Schedule and facilitate care plan conferences.
• Facilitate monthly quality management meetings.
• Manage care planning processes.

• Nursing Degree from an accredited program or college/university.
• A current, unencumbered, active Ohio LPN or RN license.
• Ability to pass a background check and pre-employment drug screening.
• Proficient understanding of current federal, state, and local standards, regulations, and guidelines that govern long-term care facilities.
• Knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement.
• At least one year of experience in long-term care in MDS.
• RAC-CT preferred.
