Home Care Network, Inc. is seeking a Registered Nurse (RN highly preferred) or Licensed Practical Nurse (LPN w/ several years of experience) to provide Home Health Private Duty Nursing (PDN) services in and around Lancaster, OH (Fairfield County) for a loving young adult male patient with an amazing family (incl. active parental caregivers + additional HCN in‑home care team).

• Minimum of 1 year experience in both vent care and tube feedings / G‑tube care expertise (2+ years preferred)
• Minimum of 2 years of experience in paid home health or visiting nurse setting (home health agency experience a plus)
• Strong medication administration skills

• Ventilator & Tracheostomy Management
• G‑Tube & Enteral Feeding Care
• Medication Administration
• Follow physician orders and established Plans of Care
• Excellent clinical documentation
• Communicate patient changes to the RN Case Manager + Care Team members

• Active Ohio RN or LPN license

• Reliable transportation and punctuality
• Active CPR and First Aid certificates highly desired (for speed to start care), but agency will provide renewal/recertification if required.
• Good interpersonal communication skills – written and verbal.
• Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail‑oriented (in care + documentation), accountable, friendly and accommodating, respectful.
• Ability and willingness to successfully perform all required duties including essential physical demands (listed below).
• Current driver's license or state , plus reliable transportation, are required.
