HCN is seeking a Registered Nurse (RN highly preferred) or Licensed Practical Nurse (LPN w/ several years of experience) to provide Home Health Private Duty Nursing (PDN) services in and around Lancaster, OH (Fairfield County) for a loving young adult male patient with an amazing family (active parental caregivers + in-home care team).

• Minimum of 1 year experience with vent care and tube feeding / G-tube care (2+ years preferred)
• Minimum of 2 years of experience in paid home health or visiting nurse setting (home health agency experience a plus)
• Strong medication administration skills

• Ventilator & Tracheostomy Management
• G-Tube & Enteral Feeding Care
• Medication Administration
• Follow physician orders and established Plans of Care
• Excellent clinical documentation
• Communicate patient changes to the RN Case Manager + Care Team members

• Active Ohio RN or LPN license
• Minimum of 1 year experience with vent care and tube feedings / G-tube care expertise (2+ years preferred)
• Minimum of 2 years of experience in paid home health or visiting nurse setting (home health agency experience a plus)
• Strong medication administration skills
• Reliable transportation and punctuality
• Active CPR and First Aid certificates highly desired; agency will provide renewal/recertification if required
• Good interpersonal communication skills - written and verbal
• Desirable traits: reliable, honest, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful
• Ability and willingness to perform all required duties including essential physical demands (listed below)
• Current driver s license or state , plus reliable transportation, are required

• LPNs: $28 - $32 per hour
• RNs: $35 - $40 per hour

Compensation is based on employee qualifications and certifications, experience, skill specializations, positive employment history, and overall client & team fit.

A successful PDN at Home Care Network, Inc., performs skilled nursing functions in accordance with the laws and guidelines of the agency and professional standards. The LPN/RN delivers care to clients as delegated by the RN Case Manager.

RN Case Manager;
Clinical Administrator/Director of Nursing

• Provides professional nursing care under the direction of a Registered Nurse; communicates status/condition changes to the RN per agency guidelines
• Provides skilled nursing visits as directed by the RN Case Manager; follows the care plan established by the RN
• Recognizes and reports changes in client condition to the Case Manager and physician
• Communicates information in a timely and complete manner; documents in the clinical record
• Scope includes personal care, assessment, medication monitoring/administration, client teaching, emergency intervention, and nursing services per scope of practice
• Licensed personnel may perform venipuncture for obtaining blood samples where applicable
• Promotes safety and a safe environment for clients and coworkers
• Adheres to safety/infection control practices and agency policies
