- Per Diem Position at St Johnsbury Center for Living and Rehabilitation in Lancaster, New Hampshire, United States. Our mission is to Revolutionize Care Delivery™, and we're seeking a dedicated and compassionate Physical Therapist to help us achieve this goal.

• Assessing patient needs and developing individualized treatment plans

• Implementing and modifying treatment plans as needed to achieve patient goals

• Providing direct patient care, including but not limited to, exercises, modalities, and manual therapy

• Communicating effectively with patients, families, and healthcare team members to ensure comprehensive care

• Participating in wound care and pressure ulcer prevention programs

• Collaborating with other healthcare professionals to achieve optimal patient outcomes

• Degree in Physical Therapy from an accredited program

• Current licensure in the state of Vermont

• Ability to work independently and as part of a team

• Excellent communication and interpersonal skills
