Wellness Council of Indiana - Indiana Healthy Communities Perry County Indiana Healthy Communities Community Coordinator Position Description

Summary:

The Community Coordinator is responsible for coordinating implementation of the Perry County Indiana Healthy Communities Action Plan. This position will support coalition activities, community engagement, project implementation, reporting and administrative coordination related to community resources, transportation, housing stability, mental health and overall community well-being.

Essential Duties and Responsibilities:
  • Overall day-to-day responsibility for coordinating coalition activities and implementation of the Perry County Indiana Healthy Communities Action Plan.
  • Coordinate coalition meetings, steering team meetings, work groups and community partner convenings.
  • Prepare meeting agendas, notes, action items and follow-up communications.
  • Maintain regular communication with coalition members, community partners and the Wellness Council of Indiana.
  • Track project timelines, action plan goals, grant deliverables and implementation progress.
  • Coordinate the Community Resource Assessment and Service Mapping process and support maintenance of the Perry County Community Resource Hub.
  • Coordinate planning, partner engagement, implementation and evaluation of the Transportation Pilot Program.
  • Support community partners in completing the Housing and Homelessness Needs Assessment.
  • Assist the coalition in identifying opportunities to improve access to mental health and other community services.
  • Build relationships with residents, employers, healthcare providers, schools, nonprofit organizations, faith-based organizations, local government and other community stakeholders.
  • Represent the coalition at community meetings, events and outreach activities.
  • Collect and organize project data, community feedback and supporting documentation.
  • Prepare progress reports, summaries and other materials for coalition leadership, WCI and funding partners.
  • Assist with grant reporting, evaluation and sustainability planning.
Nonessential Duties/Responsibilities:

Other duties and projects as assigned and as necessary to support the Perry County Indiana Healthy Communities initiative.

Job Requirements:
  • Associate’s or bachelor’s degree in public health, community development, social work, human services, nonprofit management, health administration or a related field preferred. Equivalent experience may be considered.
  • Two or more years of experience in community engagement, project coordination, public health, nonprofit organizations or coalition-based work preferred.
  • Strong organizational, project management and time-management skills.
  • Excellent written and verbal communication skills.
  • Experience facilitating meetings and building collaborative partnerships.
  • Ability to work independently while managing multiple priorities.
  • Strong computer skills, including Microsoft Outlook, Word, Excel, PowerPoint and virtual meeting platforms.
  • Knowledge of Perry County organizations, services and community needs preferred.
  • Valid driver’s license and reliable transportation.
Duration:

This is a part-time, grant-funded position anticipated to average approximately 20-25 hours per week. The position is project based and will remain in place for the duration of available funding. The final engagement structure is to be determined and may include a contractual relationship with the Wellness Council of Indiana or funding through a subgrant to a local organization.

Direct Reports:

The incumbent is not anticipated to have direct reports and therefore will have no formal supervisory responsibilities.

Personal Work Relationships:

Ability to work well with others and within a team, both externally and internally. The position requires regular collaboration with coalition members, community partners, residents, WCI staff, local government, employers, healthcare providers and nonprofit organizations.

Physical Effort:

Office, remote and community-based environment. Standing, sitting, walking, speaking, hearing, manual dexterity and occasional lifting or transporting of meeting materials weighing up to 25 pounds.

Travel:

The position will require regular local travel throughout Perry County for coalition meetings, partner engagement and community events. Occasional evening meetings and limited travel outside Perry County may be required.
